Telephonic Registered Nurse (English/Spanish)

Inovalon is actively recruiting Clinical Communication Center Nurse(s) (RN) to telephonically coordinate healthcare screenings or assessments and educate patients on chronic conditions.

Responsibilities:

• Complete telephonic health risk assessments or screenings that facilitate patient stratification and the identification of appropriate evidence-based interventions designed to improve patient health outcomes;
• Based upon ongoing reviews of accumulated patient information, implement interventions and services according to pre-established protocols and care plan guidelines designed to achieve chronic care program patient goals;
• Communicate professionally with patients, providers, and caregivers to obtain or provide information, including completing telephonic health counseling and education calls;
• Provide disease-specific education in accordance with corporate policies, procedures and program content.
• Advance knowledge base and communication skills through training, mentoring and observation.
• Maintain compliance with Inovalon policies and procedures and client organization contractual agreements
• Adhere to all confidentially and HIPAA requirements as outlined within Inovalon policies and procedures and your Employee Agreement in all ways and at all times with respect to any aspect of the data handled or services rendered in the scope of work; and
• Fulfill those responsibilities and/or duties that may be reasonably provided by Inovalon for the purpose of achieving operational and financial success of the Company.

• Registered Nurses (RN) with active licenses;
• Bilingual (English/Spanish) required;
• Familiarity with chronic care conditions offered by Inovalon to client organization;
• Demonstrated phone communication experience with simultaneous ability to document findings in a computer and the ability to manage and meet deadlines;
• Highly computer literate and detail oriented;
• Professionally skilled in personal presentation, verbal, and written communication; and
• Meticulous organizational skills
